Senior Social Worker - Disability Service - Wandsworth - Up to £43,887 per annum

We are currently looking for a highly qualified Senior Social Worker to to join a 0-25 Disability Service based in Wandsworth.

The successful candidate will play a central role in delivering high-quality and responsive social work across children's and adults' services (up to 25).

Responsibilities of this Senior Social Worker role include:

* Assess, plan and review disabled young people for short breaks, wellbeing and safeguarding purposes under the Care Act
* Work collaboratively with allied health and social care practitioners, bringing together and coordinating the team around the young person to ensure a multi-disciplinary approach to safeguarding, as well as assessment, planning, implementation and review (APIR)
* Act as the lead professional in statutory cases - including transition (Care Act 2014) assessments and support planning, mental capacity work and safeguarding adults - while undertaking the full range of social work tasks with disabled young people and their families

Essential requirements of this Senior Social Worker role include:

* A relevant social work qualification and Social Work England membership
* Passed your ASYE year
* Creative planning & assessment skills
* Experience of working across voluntary and statutory agencies
* An excellent communicator
* Understand the needs of children and young people
* Applied knowledge of relevant legislation and guidance
