Social Worker - Adults Community - Swindon - £28ph 

Our client in Swindon is looking for a qualified Social Worker to join their Adults Community Team. The successful candidate will be HCPC registered and have must have post qualified experience working with both mental health and adult learning disabilities within a community setting.  

The right candidate should have worked within the Mental Capacity act and will have had Care Act training since the change to the Care Act (2014). The post is within a small, friendly and supportive team with great stability, allowing the right individual to enhance their skills further. Caseloads are evenly distributed between the team and the role has a competitive pay rate.

As an experienced Adults Community Social Worker, your duties will include:
  • Assessing and reviewing needs under the eligibility criteria of adult social services    
  • Co-ordinating allocation of safeguarding referrals and monitoring progress.
  • Designing individual care packages with the service user, family and or carers within a community setting
  • Experience with duty type work
  • Requesting funding for packages of care via the resource allocation group by means of weekly presentation and negotiation
  • Undertaking safeguarding investigations within the community and residential settings
  • Chairing professional and formal meetings in relation to service user eg: multi-disciplinary meetings and case conferences
  • Contributing to team duty rotas to provide prompt and emergency support as required
  • Maintaining professional knowledge and skills via continuous professional development, reflective practice, and training and relevant post qualifying awards under the PQ framework
  • Maintaining knowledge of relevant research, government guidance, legislation and best practice in my specialist areas of practice
  • Contributing to the continuous improvement and development of adult services including   providing oversight and support to other professionals
  • Student mentor offering day shadowing experiences within the team and supporting Bournemouth University with the selection of future degree/masters candidates as a member of the interview panel
  • Work within the HCPC codes of practice
As an Adults Community Social Worker, you MUST be:       
  • HCPC registered    
  • Eligible to work in the UK  
  • Available to work 37 hours a week    
  • A full UK license business insurance 
  • Good with IT systems
