Social Worker - Adults Learning Disabilities  - Reading  - £28 - £30p/h 

Our client in Reading is looking for a social worker to join their Learning Disabilities Team. The ideal Social Worker will be registered with the HCPC and have some experience working with adults. The successful social worker will also have previous post qualified experience of social work, in local government, voluntary, independent sector and hospital experience. The post is within a team that offers a lot of support and is a great opportunity for the right individual to gain more skills. You will also have full access to all training opportunities as a permanent member of staff would

 As a Learning Disabilities Social Worker you will be responsible for;  

  • Carrying out reviews and assessments of the needs of people with learning disabilities against criteria for service
  • Devising cost appropriate support plans then implement them
  • Working to ensure the protection of vulnerable adults
  • Promoting service users choices, control and independence
  • Supporting service users to live safely and independently in their own homes
  • Undertaking duty and assessment tasks on a rota basis
  • Assisting service users with direct payment
  • Arranging appropriate care and placing service users in nursing or residential homes if required
  • Assessing and reviewing existing service users placed in care homes
  • Reviewing residential, nursing home  or day centre placements
  • Working effectively with other professionals 
  • Ensuring assessments are accurate and service users are able to self-direct the assessment process and make informed decisions
  • Using local knowledge and professional experience to make decisions that uphold autonomy whilst recognizing the social work role for safe intervention
  • Playing a role within a range of multi-agency teams to ensure co-ordinated, appropriate assessments and support that remain person-centred and enable. 

As a Learning Disabilities Social Worker you should have...  

  • Experience of working with adults with learning disabilities
  • Good understanding of Safeguarding and how it applies
  • Experience of working with people within supported housing/living situations 
  • Negotiating with providers and families 
  • Putting in place personalised budgets 
  • Identifying alternative preventative services 
  • Working up individual support plans that identify support/care hours needed to meet specific outcomes
  • HCPC registered
  • Eligible to work in the UK
  • Car Driver
