Social Worker - Adults Hospital Discharge - Surrey (St Peters Hospital) - £27 per hour 

Surrey are looking for an experienced social worker to join their discharge team to undertake assessment of risk involving safeguarding of vulnerable adults/carers and their families across a range of settings. The successful candidate will have sound hospital discharge experience with excellent written communication skills as report writing is a key aspect of this role, primarily involving the completion of DSTs and participating in Continuing Health Care Assessments.

Our client is looking for someone to with experience of:

  • Working with vulnerable adults in a hospital setting
  • Have sound knowledge of the new Care Act
  • Safeguarding investigations
  • Mental Capacity Assessments
  • Complex caseloads
  • Ideally have experience using Mosaic
  • Excellent IT skills
  • Working in a fast paced environment
  • Facilitating safe and timely discharges from hospital
  • Respond quickly to referrals coming in
  • Working with patients aged 18+ 
  • Safeguarding vulnerable adults
  • Arranging short term care plans in line with the Care Act 

As a Hospital Discharge Social Worker, you will:

  • Work with vulnerable adults in a hospital setting
  • Carry out safeguarding investigations
  • Complete Mental Capacity Assessments
  • Carry a complex caseloads
  • Carry caseloads and meeting agreed targets

As a Hospital Discharge Social Worker, you MUST be:

  • HCPC registered
  • Eligible to work in the UK
