Social Worker - Joint Assessment and Discharge Team - London, Barking and Dagenham - £28ph 

My client in London, Barking and Dagenham is looking for an experienced social worker to join their JAD (Joint Assessment and Discharge Team) in a hospital located in London. The hospital team is a well-managed team, well-connected and need someone who are ready to hit the ground running.

As a Hospital Discharge Social Worker, you will have experience of:

  • Working with vulnerable adults in a hospital setting
  • Safeguarding investigations
  • Mental Capacity Assessments
  • Complex caseloads
  • Carrying case loads and meeting agreed targets
  • Working in a fast paced environment

Specific job duties within the team:

  • Multidisciplinary working alongside medical professionals
  • Ensuring care from hospital discharge to their own home
  • Dealing with sections 2s and 5s
  • Ensuring timely discharges within the 72 hour frame
  • Timely and appropriate safe hospital discharge carried out daily
  • Helping service users, looking at their care packages to see if they need increasing or putting ones into place
  • Appropriate discharges into nursing or residential homes
  • FACs criteria to carry out assessments
  • Support service users to get back to independence and living safely in their homes

As a Hospital Discharge Social Worker, you MUST be:

  • HCPC registered
  • Eligible to work in the UK
