Social Worker - Integrated Disabilities - Lambeth - £28 per hour

I am recruiting for a social worker to join my client in Lambeth to work on Integrated Disabilities cases. Candidates for this vacancy must have significant experience in either learning disabilities, physical disabilities, or both. You will be required to complete reviews, assessments and provide supportive packages of care. This role will require ability to work autonomously.

Duties as a Social Worker on Integrated Disabilities:

  • To carry out an assessment or enquiry and plan responses with an adult, or their representative or advocate if they lack capacity, to safeguard them from abuse, neglect or exploitation, building effective relationships with individuals and families as appropriate. 
  • To be the lead professional in collaborative work in situations where a social work perspective is needed to inform or drive decision-making, ensuring that a legally based, human rights perspective is applied to when and how interventions are made in the lives of individuals, families and communities experiencing complex social and family problems
  • To take a lead on community development to assess, identify and maximise the strengths or assets of individuals, their families and their communities
  • Act as the key person in case managing a range of service users with learning disabilities and their carers who require additional support or guidance in order to identify their needs 
  • Lead on and complete holistic assessments and reviews of the social care and general health needs of individuals with learning disabilities by working in line with the Council's 'Promoting Independence' commitment. 
  • Be responsible for all aspects of case management including safeguarding and mental capacity assessments. 
  • Adhere to the standards of proficiency set by the Health and Care professional Council so that registration can be maintained and learning or development needs are duly accounted for in supervision and appraisals. 

You MUST be:

  • HCPC registered
  • Eligible to work in the UK
  • Have significant post-qualified experience in disabilities
