Service Manager - Joint Assessment and Discharge Team - Havering - £38 to £50 per hour (DOE)

A service manager is required to join the Assessment and Discharge Team in Havering. The successful service manager will lead and manage the integrated Joint Assessment and Discharge (JAD) Service to ensure effective collaborative working with interdependent teams, such as BHRUT Therapies and Redbridge Hospital Social Work Team.

Responsibilities of the Service Manager:

  • To provide operational management for the integrated Joint Assessment and Discharge Service
  • To provide strategic and professional leadership, including professional development and competencies for respective practitioners
  • To manage performance against JAD targets/objectives and to have regard to that of individual organisations
  • To analyse trends e.g. frequent attenders, borough trends, reduction in bed use, increase in community care packages.
  • To be accountable to Barking & Dagenham (as the lead organisation) for the delivery of key performance indicators and budgetary control
  • To provide a senior operational interface with other senior staff within the health and social care system
  • To provide dynamic leadership of all staff to develop a shared understanding of, and commitment to, the vision and strategic aims of the JAD service 
  • To report to the four agencies who have, made available, or seconded staff, to the JAD Service through a JAD Steering Group, chaired by the lead organisation,  Barking & Dagenham
  • To be responsible for the delivery of the personalisation agenda for service users from Barking & Dagenham and Havering 
  • To ensure the consistent application of relevant eligibility criteria Fair Access to Care Services, Continuing Health Care and Rehabilitation
  • To be accountable for committing expenditure on packages of care and placements
  • To ensure service users, including self-funders, receive the appropriate level of support to plan their own packages of care and are able to exercise choice across a range of options
  • To ensure arrangements are in place to provide care and support to people who lack capacity, or where there are safeguarding concerns 
  • To ensure strategic and operational relationships are productive across a wide range of stakeholders, ensuring JAD aims and objectives are understood by all partners

Essential Requirements of the Service Manager:

  • HCPC registration
  • Eligibility to work in the UK
