Social Worker - Continuing Health Care - Hertfordshire - £26 per hour
Hertfordshire County require a Social Worker to cover Continuing Healthcare (CHC) assessments in older people and sensory services area of Herts Valleys based in Stevenage.
Candidates for this vacancy must be experienced in post-qualified roles.

About the role:
- These roles are required to undertake some specific work around Continuing Health Care (CHC).
- The assignments will be for an initial period of 3 months with a view to be ongoing.
- Experience within CHC would be ideal but not essential.
- Experience of completing detailed work/assessments in a timely way would be essential.
- To assist the EIT teams in completing the assessments within tight time frames. They will need to have sound Knowledge of both the Care Act 2014 and the National Framework for Continuing Healthcare 2012.
- HCPC registration
