Service Manager - Adult Joint Assessment and Discharge - Havering - £38p/h

We recruiting for a service manager to lead and manage the integrated Joint Assessment and Discharge (JAD) service in Havering, London. As a service manager we are looking for the successful candidate to be able to ensure effective and collaborative work with interdependent teams such as the BHRUT Therapies and Redbridge's Hospital Social Work team. 

This is to be done under the supervision and guidance of Barking and Dagenham as the lead organisation. Previous management experience is essential for this role.

As the JAD Service Manager you will be expected:

  • Provide operational management for the integrated service
  • Show professional leadership
  • To manage performance against JAD targets/objectives
  • To ensure that the following overall aims and objectives of the JAD service are met
  • To facilitate safe return home through collaborative working
  • To provide the integrated health and social care support required to discharge patients
  • To identify end of life patients who wish to be looked after at home and ensure that they receive expedited discharge with the right support
  • To minimise delays arising from problems with inter-agency liaison
  • To focus decision making with the service user at the centre of processes
  • To analyse trends
  • To provide a senior operational interface with other senior staff within the health and social care system
  • To provide dynamic leadership of all staff to develop a shared understanding of, and commitment to, the vision and strategic aims of the JAD service

You MUST have:

  • HCPC registration
  • Eligibility to work in the UK
  • Previous management expreience
