Social Worker - Leaving Care - Tameside - up to £26.88p/h

An exciting opportunity has arisen for an experienced Social Worker to join Tameside's busy, but supportive leaving care service. As a children's social worker your duties will include completing pathway plans, as well as promoting independent living skills for people aged between 16 – 21 years leaving care.

The successful candidate will have experience of undertaking these pathway planning processes in support of looked after children and care leavers and be able to evidence achieving good outcomes in support of young people undertaking the transition to adult life. The successful candidate will also have experience of joint homelessness assessments with a good understanding of the duties placed on the local authority when working in partnership with housing services.

Your duties as a Leaving Care Social Worker will include:

  • Completing pathway plans
  • Promoting independent living skills for people aged between 16 – 21 years leaving care
  • Achieving appropriate accommodation, training, education and employment for the service user
  • Managing a high caseload
  • Statutory reviews
  • Employment assessments
  • Duty work
  • Managing the leaving care grant

You need to be:

  • HCPC registered
  • A qualified social worker
  • Eligible to work in the UK
