QSW - Social Worker - Hospital Discharge - Hackney - £28 per hour

We currently require a Social Worker with significant hospital discharge experience to work in a Hospital Team in Hackney.  

This role will be based within Homerton University Foundation Trust with the Integrated Discharge Service to facilitate hospital discharge and contribute towards delayed transfer of care performance. Additionally part of the role requires regular review of interim bed admissions and timely discharges for one of our placement facilities.

Responsibilities as a Hospital Discharge Social Worker:
  • Discharge planning 
  • Attend board rounds 
  • Assessing Care/Reablement packages/24 hr emergency placements 
  • DST/Continuing healthcare knowledge useful 
  • Care Act understanding 
You MUST be:
  • HCPC registered
  • Eligible to work in the UK
  • A full UK license holder
