Social Worker - Adults Community - Prestwich - £28,000 Per Annum

The main aim of this role is to provide professional social work support and undertake assessments of adult members of the Community in accordance with good professional practice and organisational policy and procedures. Where appropriate this will require close co-operation with Local Authority Social Work staff and a willingness to work according to agreed procedures. This will involve fulfilling the role of Community Care Assessor and/or Care Manager and provide ongoing casework support where appropriate.

Main duties and responsibilities:

* Undertake needs led Community Care Assessments of service users and carers
* To act as a Care Manager by arranging, monitoring and reviewing packages of support provided to service users and carers following assessment
* To implement and manage the Community Care Assessment process in accordance with agreed procedures between the Federation of Jewish Services and the Local Authority Social Work Teams
* To carry and prioritise a workload in accordance with the requirements of the organisation at any given time
* To provide emotional and practical support to service users, carers and their families in accordance with good professional social work practice and the organisation's policies and principles
* To act as an advocate for service users and carers to ensure they receive a religiously/culturally appropriate service
* To maintain up to date records of work undertaken and produce statistics in accordance with the organisation's requirements
* To convene and/or attend case reviews/discussions/conferences
* To produce written reports when required by your line manager or other agencies
* To be available at the office at specified times to provide a duty service, seeing service users, accepting referrals etc.
* To contribute to the supervision and support of students as part of the Federation of Jewish Services' commitment to student placement
* To develop awareness of available community resources and legislation which will have an impact on social work practices
* To develop a close working relationship with Local Authority Social Work teams and where appropriate an awareness of their procedures and resources
* Work closely with other organisations, both statutory and voluntary and ensure high levels of liaison and communication
* To provide support to other projects within the Federation of Jewish Services at the discretion of your line manager
* To liaise regularly with other projects within the Federation of Jewish Services that provide support to service users and carers

Essential Requirements:

* Social Work England registration
* Eligibility to work in the UK
* Post-qualified experience
