Social Worker - Hospital Discharge

Social Worker - Hospital Discharge - Warrington - Up to £26.88 per hour

An experienced Adults' Social Worker is required to join the Hospital Discharge Team in Warrington. The successful Social Worker will have some experience of working within a hospital discharge team as well as working within a MDT, as you will be working closely with a number of health professionals.

As a Social Worker Social Worker will be expected to:

* Work with AADs (Admission Avoidance and Discharge to assess) sub team
* Discharge patients from the ward and follow up in the community for up to six weeks
* Carry out safeguarding or placement work that arises with this client group
* Attend morning meetings and feedback to the sub team on your progress with assessments
* Complete 2014 Care Act assessments
* Provide information, advice and/or support as needed

Essential requirements of this Social Worker role:

* Social Work England registration
* Social Work qualification with post-qualified experience
* Eligibility to work in the UK
* A full UK license
