Social Worker - Hospital Discharge

Social Worker - Hospital Discharge - Hackney - £42 per hour

We are currently recruiting for an experienced Social Worker to join a Hospital Discharge Team in Hackney.

The successful Social Worker will be responsible for supporting safe discharges from the borough's local hospital, as well as developing and maintaining links with other multi-disciplinary teams from a number of London wide hospitals.

As a Hospital Discharge Social Worker will be expected to:

* To promote and maintain independence for vulnerable adults
* Work with AADs (Admission Avoidance and Discharge to assess) sub team
* Discharge patients from the ward and follow up in the community for up to six weeks
* Complete 2014 Care Act assessments
* Carry out safeguarding or placement work that arises with this client group
* Attend morning meetings and feedback to the sub team on your progress with assessments
* Provide information, advice and/or support as needed

Essential requirements of this Social Worker role:

* Social Work England registration
* Social Work qualification with post-qualified experience
* Eligibility to work in the UK
* A full UK licence
* Degree level or equivalent in Social Work
