Social Worker / RMN - Mental Health

Social Worker / RMN - Mental Health - Southall - £27 - £29 per hour

We are recruiting for an experienced Social Worker and / or Registered Mental Health Nursr to join a Mental Health Team in Southall.

In this role as a successful candidate you will work as part of the Multi-Disciplinary Team to plan and provide a package of care/treatment which best meets the specialist needs of service users. You will also be Assessing and planning nursing care requirements.

Responsibilities as a Mental Health Social Worker / RMN:

* Admitting patients for assessment.
* Organising workloads
* Visiting patients at home
* Act as liaison between different instances which were involved, hospitals, nursing homes, crisis team, friends and family, care plans, medication administration, management, management of HCAs supervision, doing visits in homes, creating care packages for special care.
* Building relationships with, reassuring, and listening to and talking to patients
* Combating stigma and helping patients and their families manage it
* Administering medication

Essential requirements:

* Qualified Social Worker and / or RMN
* Social Work England or NMC registration
* The legal right to work in the UK
* Previous Mental Health experience
