Children's Support Advisor - Young People, Leaving Care - Cardiff - £16 per hour

We are currently looking for a Personal Advisor to join a Young People, Leaving Care Team in Cardiff. The successful candidate must have experience of assessments and person centre planning, as well as care management skills. The successful Personal Advisor will also need to have team working skills, the ability to produce innovative solutions and be forward thinking to help develop the service.

They require a Personal Advisor who has the ability to liaise with other professionals, providing excellent outcomes for young people leaving care.

Duties as a Personal Advisor:

* The role of the Personal Advisor is to provide young people leaving care with advice and guidance
* To assist young people in their participation in pathway planning to ensure that their pathway plans properly reflect their individual strengths, aspirations and needs
* Make appropriate assessments of individual's needs in collaboration with the Children Looked After Social Workers and other agencies as required

Essential requirements of this Personal Advisor role:

* Experience of assessments and person centre planning
* Enhanced DBS
* Qualification in working with young people, or suitable relevant experience is desirable
* Care management skills, working actively with all partners to identify and meet individual's needs
* The legal right to work in the UK
