Liquid Personnel is currently recruiting for an Adult Social Worker to join a busy Transfer of Care Team based in Cambridgeshire.

Your main responsibilities as an Adult Social Worker:

• Specifically to work with Self funders in hospital to support rapid discharge
• Locums will need to be able to carry out care act assessments, mental capacity act assessments  and offer/give advice and support to self funders and their families – including directing to the relevant websites and other methods of imparting information  to support discharging those who are self funding.
• Remote working but must be available to come into the acutes across all the sites so must be flexible – must work together to cover annual leave/absences
• Will need to liaise with the Discharge Planning Nurse Teams as well as the managers for social care with updates on cases

What we are looking for in an Adult Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
