Advanced Practitioner – Stevenage – Adult disabilities
Liquid Personnel is currently looking for an advanced Practitioner to join our client’s team in Stevenage.
Due to the nature of this role, our client requires a Practitioner who has post qualified experience in a similar role.

Your duties as an Advanced Practitioner:
- There are many opportunities to be involved in formal decision-making forums and court processes.
- You will require detailed assessment of need (Care Act) and effective asset-based support planning, universal and voluntary services, Carers assessments, Best Interest decision making,
- You will review each support plan and the quality of the service provision, making changes in response to evolving needs and service principles.
- To carry out the statutory duties of the Directorate in relation to adults in accordance with Directorate policies and procedures.
Further requirements:
As an advanced practitioner, you will work with adults who may require social care services, and their family and carers, assessing their social care and health needs and to plan, implement, and monitor action and outcomes
What we are looking for in an Advanced Practitioner:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
