Liquid Personnel is currently recruiting an Advanced Social Worker to join the busy team at an Haringey, London based Discharge Intervention Team.

The successful candidate will act as a care coordinator and social worker, carrying out Care Act, and initial assessments using the Care Programme approach (CPA) determining eligibility, need, and the required level of care and support.

Primary Responsibilities as an Advanced Social Worker:

• Carry out complex person-centred assessments
• Work in partnership with people, other professionals and agencies and intervene to safeguard individuals, promote social change, maximise independence and provide appropriate information and advice about third sector and universal services which support positive outcomes
• Prepare well written evidence-based reports and devise care packages
• You will have the ability to apply professional curiosity and use social work theories and knowledge to formulate and test hypotheses reaching a professional judgement and decision

Further Information:

You will work closely and collaboratively with other members of the OPMHT, including other services in Barnet Enfield and Haringey MH Trust, Acute Trusts and Local Authorities. 
You must have all employment history, Qualified social worker with experience in a team with any gaps verified.

• Social Work England registration
• Eligible to work in the UK
• post-qualified experience
• Degree level or equivalent in Social Work
