As a successful Mental Health Nurse, you will be part of a team that will be delivering the NHS Long Term Plan and the Community Mental Health Framework for Adults.

Your main responsibilities as a Community Mental Health Nurse:

• To ensure that all patients have an individual risk assessment and care plan which has been created collaboratively with patient’s, and if appropriate, carer’s views; ensuring the regular review of both.
• Develop care plans based on an assessed need that are outcome-focused.
• Monitor and evaluate interventions in order to measure progress. To undertake the role of Care Co-ordination and implement CPA, were relevant to the role.
• Where necessary modifying interventions to ensure their effectiveness.

What we are looking for in a Community Mental Health Nurse:

• NMC registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Mental Health Nursing
