Liquid Personnel is currently recruiting for a Band 6 Care Coordinator to join Croydon’s Care Home Intervention Team.

Essential requirements for the role:

• Someone with excellent knowledge of dementia in the moderate to severe stage of the disease progression.
• Someone with significant prior experience of working with care homes and other private organisations that care for people with severe dementia.
• Dementia care mapping qualification, or experience completing psychological formulations in MDT.
• Someone who has prior experience using a dementia practice model such as the Newcastle model.
• Someone who is able to demonstrate they have experience using various psycho-social interventions for wellbeing in dementia.
• Having good IT skills and written English is essential, as there is a lot of GP letter-writing and report writing to GPs and care homes.

What we are looking for in a Band 6 Care Coordinator:

• HCPC or NMC registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in OT or Mental Health Nursing
