Your main responsibilities as a Band 6 Care Coordinator:

• To provide care coordination and care management to a caseload of highly vulnerable service users. 
• To involve service – users in the assessment and planning of their care. 
• To recognise and assess the contribution and needs of carers, and to provide advice and support when required.
• To use therapeutic skills within a framework of psychological understanding of complex difficulties provided by the leadership and membership of the team. 

What we are looking for in a Band 6 Care Coordinator:

• Social Work England or NMC registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work or Mental Health Nursing
