Liquid Personnel is currently looking for an Outreach Family Support Worker to join a Team in Liverpool.

Primary Responsibilities as an Outreach Family Support Worker:

• Offer support on cases that become more complex and reach level 4. Support is continued on these cases to allow a smooth transition between early help and social care.
• Provide intensive and targeted support to vulnerable children and young people. This includes bespoke packages and programmes. This also includes supporting adolescents with complex emotional and mental health problems.
• Assess risk and need through early help assessments on cases that step down from where appropriate adopt the role of Lead Professional to use common processes for children and families who require more support than universal services including the Early Help Assessment Tool (EHAT), Team Around the Family (TAF) and Lead Professional (LP) role.

Further Requirement:

As an Outreach Family Support Worker, you will Taking whole case responsibility to provide high quality, outreach and home based Early Help support to children and families using a ‘whole family’ approach (level 2 and 3) Delivering targeted support to children across age ranges (0 -25*) and their families in order to prevent the need for statutory services and to ensure families access Universal services*Between ages 19-25 relates only to Adults where the SEND Code of Practice is applicable.
 
 What we are looking for in an Outreach Family Support Worker:

• Educated to GCSE standard and hold an NVQ Level 4 or equivalent in a related field AF & C Experience (Relevant work and other experience) 
• Eligible to work in the UK.
• Hold a full UK licence.
