Liquid Personnel is currently recruiting for a Senior Social Worker to join a busy Hospital Discharge Team based in Greenwich.

Your main responsibilities as a Senior Social Worker:

• Support the discharge of Greenwich clients from acute hospital settings in the Hospital Integrated Discharge Team. Candidates
• Be experienced at completing Strength based, person centred Care Act assessments, commissioning care/placements and liaising with clients, families, and other professionals/agencies.
• Complete written work to a very high standard whilst working in a fast-paced environment and able to manage the expectations of other partner agencies to ensure safe and timely discharges. Candidate require good interpersonal skills dealing with difficult and sometimes challenging situations and be able to participate in, or chair multi-disciplinary meetings as necessary.
• Have a good knowledge of the issues facing older people and people with disabilities and a thorough understanding of the legislative framework pertaining to Adult Social Care.
• As well as understanding the duties under the Care Act, candidates will also need a good knowledge of the Mental Capacity Act as this role includes completion of a number of Mental Capacity assessments and making best interest decisions as well as having a good understanding of the Human Rights Act.
• This role also includes supervising staff, covering the duty Manager role and providing support and guidance to less experienced members of the team. Maximum pay rate in line with the London Borough Agreement This is not a hybrid working arrangement.

What we are looking for in a Senior Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
