Liquid Personnel is currently recruiting for a Social Worker to join a CHC Team for Adults based in Bradford.

Your main responsibilities as a Social Worker:

• To complete Care Act Assessments to participate in DST meetings with health professionals.
• To complete any follow up work to ensure appropriate funding, services and support plans are in place to meet service user needs.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
