Liquid Personnel is currently recruiting for a Social Worker to join a busy Joint Emergency Team/Assessment Team based in Greenwich.

Essential requirements for the role:

• To be flexible and able to meet the challenges of working in a fast paced environment.
• To have experience of completing Social Care Needs assessment for people who have complex health needs around discharge, in order support a safe discharge from the virtual ward arrangement to their home, so it can be as seamless transition for them
• To be adept at using a strength based approach to co-produce social care solutions that meet the needs of the patient.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
