Liquid Personnel is currently recruiting for a Social Worker to join a busy Integrated Discharge Service based in Hackney.

Your main responsibilities as a Social Worker:

• To complete face to face visits.
• To demonstrate person centred working and to facilitate needs based assessments using a strengths-based approach.
• To be able to work under pressure, meet tight deadlines and work as part of a multidisciplinary team.
• To be able to record to a high standard and to demonstrate evidence based work.
• Must have experience and knowledge of safeguarding and section 42 enquiries.
• Work in partnership, formulate and implement personal budgets and support plans.
• Assist and participate in the management of the duty service and carry out screening of clients to determine criteria to be supported by the service.
• To be expected to also work to the D2A (discharge to assess) pathway and complete community care act assessments

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
