Liquid Personnel are currently recruiting a Social Worker to join the Integrated Discharge Team based in Hackney.

The successful Social Worker will have experience in a hospital role and good knowledge of the Liquid Logic system.

Your main responsibilities as a Social Worker:

• Hold a caseload of patients that need a Care Act assessment to facilitate safe discharges from hospital
• Think home first – with packages of care to meet needs and assistive technology
• Working closely with all MDT members in the hospital
• To complete Mental Capacity Assessments and make best interest decisions under the Mental Capacity Act
• To support Community Assessments Officers
• To provide daily updates to IDT managers for the Criteria to Reside report
• To work flexible and pick up urgent pieces of work when needing
• Prioritising your caseload daily to meet the demands of patient flow

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
