Liquid Personnel is currently recruiting a Social Worker to join an Adults Assessments / Adults Physical Disabilities Team based in Lancashire.

Your main responsibilities as a Social Worker:

• To participate in a full and flexible manner in the provision of services to vulnerable adults with complex mental health support needs and their carers, using appropriate resources of the Council, the Health Trust, the local community, non-statutory organisations, and primary care.
• To participate in the assessment or care management service in conjunction with other professionals in the team.  To participate in the Approved Mental Health Professional rota, ensuring that an effective and sensitive service is always provided, with full documentation logged and distributed as appropriate.
• To provide a professional social work service to adults (including older people) with physical, sensory, or learning disabilities and their carers (including those who are terminally ill) with a particular focus on enquiries into allegations of significant harm(abuse).
• Working with individuals and/or their families and multi-disciplinary colleagues, providing practical assistance, advice, and information, working with community supports and liaison with other agencies, seeking the help of other professionals as necessary.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
