Your main responsibilities as a Social Worker include:

• Care Act assessments
• NHS Continuing Healthcare assessments
• Be able to undertake assessment and care management functions including presenting to Outcomes Forum for funding decisions.
• Able to work at a fast pace.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
