Liquid Personnel is currently recruiting for a Social Worker to join a busy Adults Hospital Team based in Redbridge.

Your main responsibilities as a Social Worker:

• Ensure execution of safe discharges of service users in need of care back into the community with joint coordinated working with health colleagues.
• Social Workers will hold a complex caseload of residents who are admitted to hospital and ensure that there is timely and safe discharge planned to ensure integration back in the community.
• Offer service users and their carers appropriate assessments, care and support plans and reviews in accordance with legislation, Council and Departmental Procedures and best practice.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
