More information about the role:

• This role involves reviewing, with the provider, Individuals who are already in receipt of services but who may no longer be eligible for these services. The aim is to reduce these packages of care as appropriate and upskill the Domiciliary care provider to highlight these to adult Social Care.
• The candidate will work directly with the provider, under the supervision of the team manager to identify individuals and complete joint assessments to enable the provider to develop these skills.
• The candidate must have sound knowledge and understanding of Care Act eligibility criteria.
• The candidate must have understanding of strength based practice and care planning. This will be a fast paced role and the candidate will be based at both the providers office, the Social care office and complete visits to individuals homes.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
