Liquid Personnel is currently looking for an experienced Adults Social Worker to join an Integrated Urgent Care Team in Tameside, Greater Manchester.
Due to the nature of the role, the successful Social Worker will have experience of CHC, safeguarding, risk management, and commissioning of services.

What will you be doing as a Social Worker in Tameside?

• You will complete same day, urgent assessments in the community, to reduce the risk of a breakdown in care/ admission to hospital
• You will be undertaking Care Act Assessments to device packages of support
• You will be working within a Hospital Team, where experience of discharge to assess, end of life and intermediate care is essential

What we are looking for in a Social Worker?

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
