Liquid Personnel is currently recruiting for a Social Worker to join a busy Hospital/Assessments Team based in Wandsworth.

Your main responsibilities as a Social Worker:

• You will lead and represent Adult Social Care at regular multidisciplinary meetings, complete assessments and support plans and arrange onward care on discharge from hospital, providing advice and information as required.
• The work also includes assessment of care and support needs, support planning, case management, duty, and face to face assessments.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
