3 essential requirements for the role/System knowledge required:

1. Facilitate effective assessment and support planning in accordance to the Care Act 2014 for adults with physical health needs.  
2. Be responsible for undertaking reviews of our service users and supporting them and their needs as required, within the statutory timescales.
3. To care manage a number of cases, including undertaking safeguarding investigations

What we are looking for in a Social Worker: 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK license
• Post-qualified experience
• Degree level or equivalent in Social Work
