Job Specification:

Carrying out needs led assessments, visiting clients within a hospital and community setting, liaising with other staff in Acute and Primary Health Care, Social Services and Provider Organizations to ensure that identified needs are being met in the most effective way possible. Ensure the provision of high-quality, cost-effective services to vulnerable adults.

What we are looking for in a Social Worker:

• Social Work England registration
• Eligible to work in the UK
• ASYE completed or equivalent demonstrable experience in a statutory social work setting
• hold a valid UK driving license and have access to a car (exceptions may apply under the Equality Act)
• Degree level or equivalent in Social Work
