Community Engagement Worker 18-25 – Manchester – Transitions Team

Liquid Personnel is offering a dynamic role within the Transitions Planning Team, which provides a flexible hybrid working arrangement with a 2/3 day split that adjusts according to caseloads and meetings. This dedicated team focuses on aiding young adults in their transition to adulthood, collaborating with key individuals in their lives and interfacing with statutory services in health, education, children’s social care, and the voluntary sector. The core mission is to empower these youths as they navigate a period marked by celebration, change, choice, decision-making, and challenges. As a Community Engagement Worker, your essential contribution will be to support clients aged 18-25 in accessing mental health services, thereby playing a crucial role in their developmental journey.

 

Your responsibilities will include:

  • Building therapeutic relationships with clients using a person-centered recovery approach.
  • Providing psychologically informed interventions.
  • Collaborating with the Core Locality Multi-Disciplinary Team and appropriate intensive Pathways to support clients in developing their recovery plans and promoting personal responsibility.
  • Assisting clients in accessing local community resources and statutory services.
  • Acting as the named key-worker for a number of clients.
  • Facilitating group work under the supervision of senior staff and participating in other clinical activities as required.

Qualifications and Experience

We are ideally seeking a qualified social worker with the following attributes:

  • Experience working with young people or adults with care and support needs.
  • A person-centered and strengths-based approach to practice.
  • Previous experience working with young people with learning disabilities.
  • Familiarity with relevant legislation, including the Care Act 2014, Children Act 1989/2004, Mental Capacity Act 2005, Children and Families Act 2014, Equality Act 2010, and Human Rights Act 1998.
