Liquid Personnel is currently recruiting experienced and Qualified Social Workers to join a busy Assessment Team based in Islington.

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.

Your main responsibilities as a Social Worker: 

• The post holder will be accountable to Adult Social Care Services for ensuring that adults are reviewed under the Care Act 2014 Assessment to check they continue to be eligible for services and to provide services only to people who meet these criteria
• The post holder will be involved in project work which will include three projects. This entails reviewing the top 20% of high packages of care, Homecare mobilization project and assessing residents that attend in house day Centre
• To work closely with other teams, including undertaking joint assessments, with providers, carers and external agencies to deliver outcome focused services to individuals and their families
• To fully comply with departmental policies and procedures and the Health and Care Professionals Council’s Code of Conduct for Social Workers including the capability framework.
• The post holder will be expected to carry out a holistic review’s and explore if heath funding can be applied

What we are looking for in a Social Worker: 

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK license
• Post-qualified experience
• Degree level or equivalent in Social Work
