Liquid Personnel is currently recruiting for experienced and qualified Social Worker to join a busy Hospital Discharge Team based in Lewisham.

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.

Essential requirements for the role:  

• Knowledge on the CHC process and discharge to assess pathways
• Skilled report writing and manage the recording and administration around assessment and planning.
• Identify and carry out Mental Capacity Assessments and chair best interest meetings.

Key Responsibilities:

• To support vulnerable adults and to make informed decisions and choices in their use of resources, support and services.
• Complete initial visits within 24 hours to ensure the citizen is is safe and then decide with pathway the patient should go.
• To undertake assessments and then and re-assessments in line with the care Act 2014.

What we are looking for in a Social Worker:  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK license 
• Post-qualified experience 
• Degree level or equivalent in Social Work
