Your main responsibilities as a Social Worker:

• Facilitating safe and prompt hospital discharges
• Evaluating the requirements of patients and their families
• Creating and executing care strategies
• Collaborating with healthcare experts and local services
• Offering guidance and assistance to patients and their families

What we are looking for in a Social Worker:

• Degree in Social Work
• Registered with SSSC
• Experience in hospital discharge planning
• Knowledge of relevant legislation and policies
• Excellent communication and interpersonal skills
